GLOBAL ARCHITECTURE OF THE PUBLIC HEALTH SYSTEM: FROM AN ECOSYSTEM TO A CIVILISATIONAL APPROACH

Purpose of the work is to substantiate and propose a global architecture of the public health system that integrates “One Health”, "Health in All Policies", SDGs and SDGs through the transition from an ecosystem (zoonosecentric) to a civilizational approach with a focus on non-communicable diseases as the main object of preventive management.
Research methodology. The study is based on a systemic approach to the analysis of the architecture of the public health system. A systemic analysis of international regulatory and strategic documents (One Health, Health in All Policies, social determinants of health, Sustainable Development Goals), a cross-influence method for assessing the systemic significance of SDG indicators, a case analysis of the implementation of the One Health concept in Ukraine and a comparative conceptual analysis that justifies the transition from an ecosystem to a civilizational approach in the design of the architecture of the public health system were applied.
Scientific novelty. For the first time, an integrative four-level model of the global architecture of the public health system is proposed, combining the concepts of One Health, social determinants of health, Health in All Policies and Sustainable Development Goals. The concept of “One Health-filter” is introduced as a tool for identifying intersectoral risks.
Conclusions. The proposed global architecture of the public health system, which integrates the concept of “One Health” (bioecological core), social determinants of health (socio-economic level), the strategy “Health in All Policies” (management mechanism) and the Sustainable Development Goals (normative and target framework), allows us to justify the transition from an ecosystem-based (focused mainly on zoonotic infections) to a civilization-based approach, where the main object of preventive management is non-communicable diseases (90 % of the global burden), caused by anthropogenic changes on a planetary scale. For Ukraine, taking into account the war-related environmental degradation, demographic crisis and the growth of mental disorders, the priority is to implement the “One Health Filter” tool, integrate epidemiological surveillance, environmental monitoring and veterinary safety into a single analytical and management platform using the identified TOP-5 priority tasks of the SDGs (3.d, 2.4, 6.3, 11.6, 15.5), which will ensure the transition from disease response to preventive management of civilizational determinants of health.
